This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.
How We May Use and Disclose Your Health Information
We use and disclose your protected health information (“PHI”) to provide, coordinate, and bill for your dental treatment and for the other purposes described below. The following categories describe the ways we typically use and disclose PHI without asking for your written authorization.
- Treatment: We may use and disclose your PHI to provide, coordinate, or manage your dental care and any related services. For example, we may share information about your treatment with a specialist, laboratory, or other provider involved in your care, or with a dentist covering our practice after hours.
- Payment: We may use and disclose your PHI to obtain payment for the services we provide. For example, we may send billing information to your dental or health insurance plan, or to a collection agency, so that we are paid for services rendered.
- Health Care Operations: We may use and disclose your PHI to run our practice and make sure our patients receive quality care. Examples include quality assessment and improvement activities, staff training, licensing, credentialing, business planning, and internal audits.
Your Health Information Rights
You have the following rights with respect to the PHI we maintain about you. To exercise any of these rights, please submit a written request to our Compliance Department atCompliance@mysagedental.com.
- Right to Inspect and Copy: You have the right to inspect and obtain a copy of the PHI we maintain about you, such as your dental and billing records, with limited exceptions. We may charge a reasonable, cost-based fee for copies.
- Right to Request Amendment: You have the right to request that we amend PHI you believe is incorrect or incomplete. We may deny your request in certain circumstances, and we will explain our reasons in writing if we do.
- Right to an Accounting of Disclosures: You have the right to receive a list of certain disclosures we have made of your PHI, other than disclosures for treatment, payment, health care operations, and certain other excepted disclosures.
- Right to Request Restrictions: You have the right to request a restriction on the PHI we use or disclose about you for treatment, payment, or health care operations, or to family members and others involved in your care. We are not required to agree, except that we must agree to a request to restrict disclosure to a health plan if the disclosure is for payment or health care operations and pertains to a service you (or someone on your behalf) paid for out-of-pocket in full.
- Right to Request Confidential Communications: You have the right to request that we communicate with you about your health matters in a certain way or at a certain location — for example, by contacting you only at work or by mail. We will accommodate all reasonable requests.
- Right to a Paper Copy of This Notice: You have the right to a paper copy of this Notice at any time, even if you agreed to receive it electronically.
- Right to Choose Someone to Act on Your Behalf: If you have given someone medical power of attorney or if someone is your legal guardian, that person can exercise your rights and make choices about your PHI.
- Right to Be Notified of a Breach: You have the right to be notified if a breach occurs that may have compromised the privacy or security of your PHI.

- Marketing: Most uses and disclosures of your PHI for marketing purposes require your written authorization.
